AvMed's Care Management Programs address the needs of members with chronic conditions, specialty care needs or complex life-threatening illnesses. A highly trained care team is available to help these members navigate the health care system and maximize their benefits. Each eligible member is assigned a registered nurse or case manager. Through education and individualized attention, AvMed empowers its members to take an active role in managing their health. Our case managers work with physicians to coordinate a member’s care and help them make positive lifestyle choices and educated health care decisions.
