Effective December 20th, 2011 AvMed HealthPlans transitioned from Healthways to Alere, a new Disease Management company which will continue providing care management services for your patients.

As part of the new service, Alere nurses will now provide guidance and support; including educational materials, tools, and assistance with managing your patients condition.

You may expect to receive a list of communications from Alere on behalf of AvMed Health Plans.
