View our Notice of Privacy Practices. This notice of Privacy Practices is directed to all members of AvMed’s health plans. It describes how we may collect, use, and disclose your protected health information, and your rights concerning your protected health information.

Miami Dade County/Public Health TrustWelcome Miami-Dade County and Jackson Health System/PHT Employees! Effective January 1, 2008, AvMed Health Plans will be offering Miami-Dade County and Jackson Health System/PHT two HMO options and a Point of Service (POS) plan. You will remain with your current carrier until December 31, 2007.

If you are not currently an AvMed member, you probably have many questions, most importantly, "Is my doctor in the AvMed network?" See below for instructions to view our provider networks.

We are currently updating this portal with new benefit information. You are always welcome to contact your dedicated AvMed Member Services Call Center at 1-800-68-AVMED (1-800-682-8633). This dedicated line is staffed by AvMed employees 24 hours a day, 7 days a week. Even before you become an AvMed member, our Member Service Representatives will be able to answer general questions about health and wellness programs, our formulary/medication list, or any other concerns you may have as a new member.

AvMed offers you reasonable rates, more choice, excellent service and the solid reputation of a statewide plan. When choosing coverage, we encourage you to evaluate all your options to find the plan that best suits your budget and healthcare needs. Consult the Summaries of Benefits below for plan details and conditions of coverage.
