Open Enrollment Information
The 2008 Open Enrollment for the State Group Insurance Program Plan Year
2009 will take place from 8:30 a.m. September 22, 2008, through 5:30 p.m.
Eastern Time October 17, 2008.
You may enroll:
- Online - Logon to the People First site at
https://peoplefirst.myflorida.com
any time.
- By telephone - Call the People First Service Center at
866-663-4735 (for TTY users, at 866-221-0268).
- Paper enrollment forms - Health Insurance
Enrollment/Change form.
You must complete your Annual Enrollment elections for 2009 calendar year
benefits between September 22, 2008 and 5:30 p.m. Eastern time on October
17, 2008. If you use a paper form, it must be postmarked by October 17,
2008.
If you are a newly hired employee or have just experienced a Qualifying
Status Change event, your enrollment deadline is driven by your date of
hire or the date of the event, respectively.
Provider Directories - Fall 2008
State of Florida employees can now visit any doctor in the AvMed Health Plans
network without choosing a Primary Care Physician (PCP) first. As always,
with AvMed Open Access there are no referrals required for specialist visits.
Although a PCP, or family physician, is not required, AvMed encourages all
members to consult a family physician because he or she should be familiar
with your medical history and can best coordinate your medical care with specialists
and other health care providers.
