When you are living with a chronic condition, AvMed’s highly trained disease management team works closely with you to answer health-related concerns, consider treatment options, provide condition specific education, and assist in coordinating your care.
How does Disease Management work?
We work with you to:
- Define your needs and goals
- Coordinate services with your provider(s)
- Find new tools to meet your goals
- Develop an individualized plan
- Provide you health information and education
How was I chosen for the Disease Management program?
There are lots of ways we find members may benefit from Disease Management. You have been diagnosed with or treated for a chronic condition listed above that may improve with the added support and education provided to you as part of this program. Your provider may have told you about the program. You may have been referred by another AvMed program or you can also refer yourself.
When are Disease Management services available? What are the hours of the program?
What if I need assistance after 5 pm?
You can call the Nurse on Call line at 888-866-5432; TTY: <711> to speak confidentially with a registered nurse about any health concerns. In an emergency, call 911.
Where do I find more information about the Disease Management program?
Feel free to call us at 833-609-0735; TTY: <711> Monday through Friday between 8:30am to 5:00pm to talk about our Disease Management program.
What if I’m not sure I want to be in the Disease Management program?
If you do not want to be in the program, that is okay. You can join the program in the future if you change your mind. You are able to opt-in and opt-out of the Disease Management program by calling 833-609-0735, or by sending an email to DM@avmed.org.
How can I give feedback on the Disease Management program?
Each year, members are also surveyed about satisfaction with this program. If you receive a letter or call regarding your participation in this program, we hope you take a few minutes to participate.
Healing Bodies Program
AvMed's Musculoskeletal Program (MSP), exclusively for State of Florida Employees
Our program is open to all State of Florida AvMed Members and includes education provided through regular electronic newsletters and on demand through our website, as well as, treatment options/alternatives and services to assist you with the management of musculoskeletal and other pain conditions. In addition, our newsletters and electronic resources will provide information, tools and videos about preventing pain in the first place.
Elements of our unique program:
AvMed encourages Physician(s) who treat Members to utilize less invasive, evidence-based treatment options such as physician supervised Home Exercise Programs (HEP), physical therapy, assistive medical devices, chiropractic and many other services and resources to help improve quality of life, without exposure to unnecessary and risky procedures.
Our program provides support based on industry leading medical evidence-based best practices. This ensures you will be guided to the best treatment options in the best settings based on your individual health, medical history and specific needs. These options range from educational resources to tips and tools, and when appropriate Complex Case Management programs.
