Non-participating providers must register with Availity to:

• Check Eligibility and Benefits
• Claim Submission and Status
• Authorizations

Submission Requirements for Claims

Non-Participating Providers must register and update tax information with AvMed Health Plans using the Non-Participating Provider Registration Form.  

Claims received without receipt of the Non-Participating Provider Registration Form first will be rejected. Once confirmation has been received that the enrollment form has been processed, claims can be resubmitted.

If this is your first time submitting a claim through this method, include a completed Form W-9 with your submission. Claims received without the required W-9 may be rejected and may need to be resubmitted, which could delay payment.
