AvMed Group Plan Details

Looking for your health plan? Here are some of our plans that many employers offer to their employees like you.

Benefits and perks can vary by plan. Many companies even customize packages for their employees or offer a few different choices. Find summaries of plan options below and talk to your HR rep with questions about enrollment or other details.

AvMed Small Group Plans (2-50 employees)

AvMed Elect (HMO)

AvMed Elect plans provide access to a broad network of providers throughout 52 Florida counties. Designed around patient-centered care, these plans emphasize the importance of coordinating care with a Primary Care Physician (PCP). Elect Members also have access to a network of specialists and facilities with no referral required.

AvMed Flex (POS)

AvMed Flex plans provide access to a select network of providers in 22 Florida counties. These plans emphasize the importance of coordinating care with a Primary Care Physician (PCP) and access to specialists without a referral requirement. Flex also includes an out-of-network benefit which allows access to non-AvMed providers nationwide.

AvMed Elite (POS)

AvMed Elite plans provide access to a broad network of providers throughout 52 Florida counties. These plans align the Member and PCP in a trusted partnership; working together toward a WELLfluent™ life without a referral requirement. Elite includes an out-of-network benefit which allows access to non-AvMed providers nationwide.

AvMed Large Group Plans (51+ employees)

AvMed Achieve (HMO)

AvMed Achieve plans provide access to a broad network of providers throughout 52 Florida counties. Designed around patient-centered care, these plans emphasize the importance of coordinating care with a Primary Care Physician (PCP). Achieve Members also have access to a network of specialists and facilities with no referral required.

AvMed Choice (POS)

Our most versatile product, AvMed Choice offers network flexibility and tiered cost share options. Through our partnership with PHCS (Multiplan), Choice Members have access to a nationwide network of healthcare providers without a referral requirement. As a point-of-service product, our Choice plans provide in and out-of-network benefits at different levels of out-of-pocket costs.

AvMed Aim (HMO)

AvMed Aim is available to South Florida based employers with 51-99 eligible employees and provides access to AvMed participating providers in Miami-Dade, Broward and Palm Beach counties. Aim Members also have access to a network of specialists and facilities with no referral required.

Health Savings Account (HSA)

AvMed Achieve, AvMed Choice and AvMed Aim offer high-deductible health plan (HDHP) options that can be paired with a health savings account (HSA). Get details on pairing an AvMed plan with an HSA.

ASO Account Management

AvMed Self-funded Options (ASO) are designed to offer Large Group Employers greater choice and flexibility in their health plans, and broad access to healthcare Providers, while controlling healthcare costs through competitive Network discounts and comprehensive medical management programs. We differentiate ourselves by delivering high-touch, personalized service to accommodate clients' business needs, helping them to more effectively manage their healthcare dollars.
